Information for a healthy relationship with SNE Primary Care

Office hours
We are open from 7am to 7pm, Monday through Friday.
Scheduling appointments
Appointments must be scheduled by telephone; we are often able to see patients the same day they call for most types of exams.
Canceling appointments
There is a charge of $25 for canceling an appointment without 24-hour notice. Repeated cancellations or "no shows" may result in your being discharged from care as a registered patient.
Urgent visits
We accommodate urgent visits on a same-day basis. Sick visits for existing patients are always accommodated within 24 hours, provided it is within our office hours.
Personal injury evaluations
We do not provide personal injury evaluations such as those associated with a Worker's Compensation claim or motor vehicle accident. Neither do we provide pain management treatment for personal injury patients.
Prescriptions
Please review your insurance, and the insurance we accept, to determine your prescription coverage. You can learn more on our prescription refills page.
Referrals
Referrals to a specialist depend on whether or not you are a current patient. Review our referrals page for more information.
Co-payments
As mandated by the health insurance plans, co-payments are due at the time treatment is received.
Hospital admissions
We admit to all the major area hospitals and medical centers; review our hospital admissions page for more information.
Release of medical information
You must sign an Authorization for Release of Medical Information form before any of your medical information if released to another party. This is among the forms available on our website. You may also want to review your right to privacy to understand more about the release of medical information.
