There are many things about you that we will need to know when you become a new patient. For instance, the first time you visit you will certainly be asked about your past medical history. Even if we have received your medical information from your previous primary care physician or practice, we will want to hear it in your words as well. And everything that is entered into your electronic medical record will remain absolutely confidential and secure.

For us to receive a copy of your medical history, you will have to authorize its release by your previous doctor—that is part of Federal law mandated by HIPAA regulations. Similarly, we will never release any personal medical history to any party without your written authorization; that and other forms you will need are available on this website.

Please note that we do not see patients for physical exams relating to Worker's Compensation, personal injury evaluation, or pain management; see our office policies for more details.
